| description | Heterotopic pregnancy is a situation characterized by occurrence of intrauterine live pregnancy with ectopic pregnancy). Recently with increasing use of asissted reproductive techniques (ART) incidence of heterotopic pregnancy increased from 1/30000 to 1/600 – 1/100. Diagnosis often is made by finding intrauterine live pregnancy in association with pelvic mass during transvaginal ultrasonography (TVUS). 23 years old patient who became pregnant with the aid of ART complained severe abdominal pelvic pain, vaginal spotting that began as brown colored than converted to bright red of 4 days history was seen at our clinic. Gynecologic examination revealed abdominal pain during palpation on right lower quadrant, rebound and palpable right adnexial mass. In TVUS examination one 7 weeks 4 days live intrauterine fetal pole, free fluid at Douglas pouch (5 cm in depth), dilatation, image of hematosalphinx and a hypo echoic region of 11 mm at rigth tubal ampullary region were determined. In the presence of signs that indicated intra abdominal hemorrhage an urgent laparotomy was performed. During exploration right tubal ampullary rupture, pregnancy material next to rupture and 500 cc free and coagulated blood were observed.
