description This article analyzes the state's ability to solve the endemic condition of leprosy in Brazil, considering this institution, not as an isolated and autonomous entity, but as a protagonist of the decision-making process of public policies that receives various influences, amongst them, their own organs and their historical trajectory. The goal is to identify and understand some of the reasons why leprosy remains an endemic disease in the country, despite being a curable disease with the use of a few blister packs of medicine that is freely distributed by HUS (Health Unic System). The study is based on conceptions of state capacity in its relation to public policy in the prospect of institutionalist authors. Keywords: State capacity; Leprosy; Endemic.
