Summary:Mature cystic teratomas are common benign neoplasms of the ovary. These tumors rarely undergo malignant transformation, the most frequent transformation is to a squamous cell carcinoma. Malignant transformation carries a poor prognosis. Herein, we present a case of a patient with a pelvic mass whose initial biopsy showed a benign teratoma, but subsequently underwent malignant transformation with fistulation to the small and large bowel.
