description Crossbite is a common anomaly in orthodontic patients. One therapeutic option is functional orthopedics. We present the case of a patient who went directly to the orthodontic service, with canine and molar relations of bilateral mesioclusion, negative overjet, full crown overshoot, left unilateral anterior and posterior crossbite. After preparing the study models and indicating the radiographic study, the orthodontic correction was carried out with the Klammt class III open elastic activator. He was checked every 21 days, after six months of treatment the molar and canine relationship was corrected, his facial aesthetics improved, but even without occlusion in the lateral sectors, the treatment with the activator was continued until the permanent dentition was completed. After two years of treatment, a correct occlusal relations was achieved in the three senses of space and adequate facial aesthetics. Treatment with the Klammt class III open elastic activator, in this patient, allowed the molar relationship to be corrected, a 1/3 crown overgrowth and a 3 mm overjet were prepared, and the unilateral left anterior and posterior crossbite was eliminated, with proper facial aesthetics.
