In this study, we determined the frequency of back pain in each trimester of pregnancy and investigated its contributing factors. Materials and Methods: This cross-sectional study was carried out in 2015 among 514 pregnant women who attended the prenatal clinic of Shaheed Mostafa Khomeini, Tehran, Iran. The data collection tools consisted of a maternal demographic information checklist and a questionnaire for assessing the status and severity of back pain. To analyze the data, Chi-squared test, independent t-test, and multivariate logistic regression were run in SPSS, version 18. P-value less than 0.05 was considered statistically significant. Results: Out of the 514 women, 355 (66.9%) had low back pain, 100 (24.09%) of whom had this pain before pregnancy. Back pain was more prevalent in the third trimester and there was a significant difference in the prevalence of low back pain among the trimesters (P=0.03). Based on the multivariate logistic regression analysis, maternal age, gestational age, and duration of standing were significantly linked to low back pain (P<0.05). However, body mass index, gravidity, route of delivery, anesthesia, and occupation were not significantly associated with low back pain (P>0.05). Conclusion: There is a high risk of low back pain during pregnancy,and among young mothers, those who stand for long durations are more prone to this problem.
