Open reduction is the appropriate management in such cases. In this statement, we present unusual isolated acute irreducible 5th toe proximal interphalangeal joint dislocation of a nine years old boy who underwent open reduction and Kirschner wires (K-wires) fixation of the 5th PIP joint due to inter positioning of the distal part of the proximal interphalangeal joint through the medial capsule preventing the reduction.
