Summary:A 44-year-old man presented with a history of crippling rapid palpitations. On evaluation, his ECGs showing incessant atrial tachycardia with variable AV conduction and LBBB during 1:1 AV conduction. Echocardiography showed right atrial (RA) dilatation with distortion of the tricuspid valve annulus. The RA dilatation was mainly because of giant RA appendage aneurysm; this was subsequently confirmed and delineated by transesophageal echocardiography and MRI. Limited aneurysmal resection with tricuspid valve ring annuloplasty was performed, and RA diathermy MAZE was carried out.
