Summary:We present a patient with a subarachnoid hemorrhage (SAH) on the computed tomography and electrocardiogram changes compatible with myocardial infarction. SAH is a medical emergency, but it can be initially misdiagnosed. Diagnosis may be delayed due to atypical presentations of SAH and this may endanger the life of the patient. Electrocardiogram abnormalities have been described previously in SAH and may obscure the correct diagnosis.
