description Objective – to evaluate of endoscopic surgical treatment of patients with carpal tunnel syndrome in comparison with direct decompression of the median nerve. Materials and methods. For decompression of the median nerve in the carpal tunnelis selected endoscopic technique and tools of K.G. Krishnan. The detailed technique of endoscopic intervention through the carpal approach is given. The results of 104 patient’s treatment by endoscopic and open methods with carpal tunnel syndrome were analyzed. Patients were evaluated prior to surgery, on day 7 and 30 after surgery. Results. Regression of neuropathic pain, according to the questionnaire BCTQ was observed in all operated patients. Functional out come in the postoperative period worsened on day 7, but in a month later under went surgery patients reported significant improvement in hand function, which corresponds to a change in the control electroneuromyography and ultrasound scanning of the median nerve. Conclusions. Endoscopic technique is the method of choice in a surgical treatment of carpal tunnel syndrome patients. Because of minimally invasive approach and sufficient carpal tunnel visualization, it can be successfully used in routine neurosurgical practice. Treatment results indicate the effectiveness of endoscopic techniques and fast rehabilitation in operated patients.
