description Background: Spontaneous external brain rupture is a rare complication in patients with untreated or neglected hydrocephalus. This forms diverticular fistula with ventricle, brain parenchyma, subgaleal space and skin. Case description: This paper presents a case of a 6-month-old infant complicating with external rupture of brain in post myelomeningocele (MMC) repair hydrocephalus who was neglected to get treatment. Discussion: Despite the advancements in investigations and management of hydrocephalus, most patients in developing countries come with big hydrocephalic head. Hence, management of hydrocephalus should start from prevention and creating awareness to the society about the condition and the knowledge that it is treatable. Most importantly treating hydrocephalus should be timely and with appropriate management protocol.
