Summary:Clinical observation of successful operative treatment of upper thoracic spine injury — unstable compression-distraction fracture-dislocation of ThIII vertebra with spinal cord anatomical disruption at this level is described. The analysis of injury mechanisms and it’s features has been made. Process of diagnostic and operative treatment is described in the article. The complex system «Stryker» (USA) with screws and hooks was used for stabilization. Measured traction of head helped to reduce the dislocation and restore spinal canal. The treatment outcome was the essential improvement: pain syndrome has regressed, patient had started to sit in a bad and to turn himself. Spiral computer or MRI tomography are the best methods for upper thoracic injury diagnostics. Posterior approach and measured traction of head can be used for treatment of this kind of trauma. Complex systems with screws and hooks can be used successfully for upper thoracic spine stabilization.
