description A 56-year-old woman presents to her primary care physician (PCP) with new-onset type 2 diabetes accompanied by comorbidities. Following diabetes guidelines, the PCP instilled insulin therapy. However, after 3 months, the patient had not reached glycemic control and reported feeling burdened by the treatment. After the clinician had an open conversation with the patient, considering the illness and the patient’s preferences, values, and context, an individualized treatment plan was developed, and better outcomes were achieved. This case highlights the relevance of shared decision-making and patient-centered care while following the best evidence-based recommendations.
