Summary:Extra-articular Synovial Osteochondromatosis (SC) is a rare joint pathology that is currently not well understood. Intraarticular and/or periarticular clusters of loose bodies can become debilitating and significantly limit function secondary to mass effect and foreign body type reactions. Surgical excision is the only curative measure at this point and intervention is further recommended due to reports of malignant transformation. We present a case of a healthy 66-year-old female with a 10-year history of a slow growing mass affecting the left lateral ankle. MRI and histopathology were consistent with exra-articular synovial osteochondromatosis and patient was managed successfully with total excision of mass. Extra-articular synovial osteochondromatosis presents a unique surgical opportunity to restore function and limit relative risk of malignant transformation. We report successful surgical outcome with immediate relief due to mass effect and significant decompression by complete excision.
