| description | This paper expounds the current situation and influencing factors of unplanned readmission of elderly patients with ischemic stroke at home and abroad, and summarizes the intervention strategies such as discharge readiness, early rehabilitation exercise, health education, follow-up and social network support, and risk prediction of unplanned readmission, in order to provide basis for improving the quality of life of elderly patients with ischemic stroke, reducing the incidence of unplanned readmission, improving the outcome of patients, and reducing the burden on families and the medical and health system.
