Summary:The immunohistochemical researches of parietal peritoneum during perforating gastroduodenal ulcer were made with the purpose to receive the grounds for the carrying out and estimation of medical efficiency of prolonged peritoneal dialysis in elderly and senile patients with diffuse peritonitis. 25 patients from 19 to 85 years were examinated. The prolonged peritoneal dialysis improved the detoxication effect and lead to the reduction of postoperative complications. Adhesive activity of peritoneum in early postoperative period during diffuse fibrinopurulent peritonitis shows the age determinism and turns out to be less expressed in patients of elderly and senile age. Possibility of carrying out of the prolonged peritoneal dialysis in elderly and senile patients with diffuse peritonitis is determined by the tendency to form less of abdominal adhesions, and the medical efficiency of this method is connected with the detoxication. Сarrying out of the prolonged peritoneal dialysis in elderly and senile patients with diffuse peritonitis results in reduction of intraabdominal postoperative complications from 34,3% to 11,1%, respiratory tract complications from 60% to 50%, wound infections from 48,6% to 44,8%.
