description Percutaneous coronary intervention (PCI) of Chronic total occlusion (CTO) is one of the major challenges in interventional cardiology. Symptomatic patients have significant improvement in quality of life after successful revascularization of CTO. Also, it has been shown to improve left ventricular (LV) systolic function, reduces the need for late bypass surgery as well as the reduction of the long term mortality. In this editorial, we review the current definition and indication for the revascularization of CTO.
