description | Introduction: One of the factors that affect aesthetics is the discoloration of the teeth. Causes of discoloration are generally divided into extrinsic and intrinsic, depending on localization and etiology. Case Description: A 18 year old male with the chief complaint of discolored upper front tooth and slight proclination. History revealed trauma in anterior teeth 10 years back. Intraorally there was no pain on percussion, the tooth was fracture of two-thirds of the incisal. Radiograph did not reveal any adverse findings.Root canal treatment was performed before and have a proper obturation. Gutta-percha was removed up to 2 mm. GIC base was applied over the stumped gutta-percha.Hydrogen Peroxide 35 % was the agent of choice. Access cavity was sealed with IRM and on the sixth day following to restored with composite resin. Discussion: Trauma to the tooth structure causes rupture of the capillaries in the pulp chamber and bleeding occurs then penetrates into the dentinal tubules resulting in a discolored tooth. The main composition used is 35% hydrogen peroxide. In this case the technique used was walking bleach. Making a cervical seal by reducing the guttapercha to a depth of 1-2 mm below the CEJ. Restoration with composite are carried out 1 - 3 weeks later, because the presence of hydrogen peroxide residue will inhibit the polymerization of the composite and reduce the bond strength to the enamel. Conclusion/Clinical Significance: Internal bleaching treatment with the walking bleach technique for discoloration due to trauma using hydrogen peroxide showed satisfactory results. |
