description | Background. The share of odontogenic maxillary sinusitis accounts for 20
to 30 % of the total number of diseases of the maxillofacial region, and therefore the
category requires a deeper study on the basis of a multidisciplinary medical organization.
Materials and methods. The analysis of statistical data of the Department of
Maxillofacial Surgery of the Penza Regional Clinical Hospital named after N.N.
Burdenko for 2018 (207 patients with odontogenic maxillary sinusitis). 226 samples
of bacteriological inoculation for flora and antibiotic sensitivity were studied. 20
primary patients with a diagnosis of odontogenic maxillary sinusitis took part in the
research in 2019, a smear was taken for flora and antibiotic sensitivity by the method
of rapid testing, and antibiotic therapy was prescribed based on the results. For
the first group, tamponade was performed using a sterile silicone balloon. According
to this method of surgical treatment, we have proposed a rationalization proposal
No.409, adopted by the Bureau of Rationalization and Invention of the Penza Regional
Clinical Hospital named after N. N. Burdenko and Medical Institute of Penza
State University. The second group received tamponade with turunda soaked in iodoform.
Results. In 2018, antibiotic susceptibility smear results were inconsistent with
empirically selected therapy in 30% of cases; 98% of patients underwent surgical intervention
in the amount of radical sinusitis with tamponade of the maxillary sinus
with turunda soaked in iodoform. In 2019, using rapid tests, appropriate antibiotic
therapy was prescribed, and 50% of patients received tamponade of the maxillary
sinusitis with a silicone balloon.
Conclusions. Appointment of appropriate antibiotic therapy from the first day after
surgery in the amount of radical sinusotomy followed by tamponade with a silicone
balloon allows you to maintain respiratory function in the postoperative period
and reduces the number of side effects. Proposed areas of application of the research
results: maxillofacial surgery. |
