Traumatic brain injury: Correlation of general patient condition, vital parameters and management at the admission to the intensive care unit, with final outcome (traumatic brain injury)

description Introduction: Traumatic brain injuries are injuries of central nervous system that may be associated with poor outcome. Mortality in patients with severe brain trauma may be as high as 30-50%. For those with moderate injuries, the death rate is significantly lower (7-9%), and related to associated trauma lesions or medical conditions. Methods: From 1.1.2015 until 31.5.2016, 43 patients admitted to our Intensive Care Unit due to traumatic brain injuries were included in observational prospective-retrospective study. Data were collected prospectively in 16 patients and retrospectively in 27 patients using an originally designed questionnaire. Observed variables were: general condition, associated injuries and vital parameters on admission, applied monitoring and treatment during first seven days of ICU stay, as well as outcome (defined as survival or a death). Correlation between some parameters and general outcome of a treatment were analyzed. Results: Death outcome was more probable in patients who were older than 60 years. Also, death was associated with presence of subarrahnoidal hemorrhage, use of mechanical ventilation and inotropes. Conclusion: This observational study confirmed that traumatic brain injury patients with poor outcome are more often with older age, subarrahnoidal hemorrhage, use of mechanical ventilation and on vasoactive medication. The only independent predictor of death in this patient population is use of vasoactive drugs.
