description The antrochoanal polyp (ACP), also known as Killian's polyp, is a benign, solid polypoid lesion which is originating from the inflammatory mucosa of the maxillary sinus, and extend into the nose, choana and nasopharynx through the maxillary ostium. Although ACP is rare, it is even more rare in children and cystic fibrosis is an important risk factor in children. Although the etiology is not completely known, allergy and sinonasal disease play an important role. They are usually seen unilateral but rarely bilateral. A 13-year-old girl presented to us with nasal obstruction, snoring and obstructive sleep apnea attacks. Endoscopic sinus surgery was performed with the aid of a transoral route in this giant ACP which is extending to the epiglottis.
