description Objective: To report a rare case of copious sialoliths from a single submandibular gland and Wharton duct and provide readers with a management consideration for similar patients. Methods: A 55-year-old-man presented to otolaryngology clinic with recurrent sialolithiasis refractory to transoral removal. Results: Upon submandibular gland excision and interventional sialoendoscopy, the patient is found to have over 40 stones within the gland and duct. Conclusion: A history of excessive stone formation may prompt earlier consideration of aggressive prophylactic treatment with submandibular gland excision to avoid symptomatic recurrence.
