Summary:Background: The management of Aneurysmal Bone Cyst of Talus (ABC) has been long debated. Being a benign tumour of the foot and ankle, the goals of therapy for ABC include symptomatic relief, prevention of recurrence and pathological fractures. There is a need to ascertain a pattern of presentation and disease progression to diagnose and administer appropriate care. Case report: A 23-year-old male with insidious onset atraumatic ankle pain. On preliminary clinical examination of the ankle, fullness was felt over the posteromedial aspect and tenderness was elicited on deep palpation with a normal range of movements. Initially, it was suspected to be Giant Cell Tumour (GCT). On fine needle aspiration cytology, it showed multinucleated giant cells. The patient underwent excision and curettage. Postoperatively, diagnosed as ABC.A 19-year-old boy with a posttraumatic swelling of the ankle. On initial radiographs, it was clear that the trauma was not the inciting factor for the swelling and after magnetic resonance imaging, it was suspected to be a haemorrhagic cyst. The patient was taken up for excision and curettage.Both patients had an excellent return to normal activities. Conclusion: An extended intralesional curettage with autologous bone grafting is an excellent management option for ABC Talus, with hydrogen peroxide as an adjuvant.
