Objective: To estimate the concordance between conventional radiology and the results
of pathological anatomy in patients with osteosarcoma.
Methods: A study of diagnostic concordance was carried out with data from medical
records, radiological reports and reports of pathological anatomy of 51 patients of both
genders, who were submitted to biopsy of bone tumor due to radiological findings
indicative of malignancy. Data were saved and analyzed in the statistical program
EPI-INFO version 3.5.1. The comparison of proportions for qualitative variables and
comparison of means or medians were done according to the normality criteria for
quantitative variables. The parameters of diagnostic validity of the clinical signs
suggestive of malignancy were determined.
Results: The number of lesions was higher in women than men (53.8% and 46.2%,
respectively). The median of age was 41 years. The most damaged bones were: Femur
49%, tibia 23.5%, humerus and radius 9.8% each one. The most frequent location
was the diaphysis. The most important radiological signs of malignancy were: Sunburst
periosteal reaction and the Codman’s triangle. For single radiology and osteosarcoma,
the sensitivity was 100%, specificity: 92.1%, validity: 94.1%, positive predictive value:
81.2% and negative predictive value: 100%.
Conclusions: The radiology offers good values of concordance for the diagnosis of
osteosarcoma. It is an important method for the approach in the presence of the
suspicion of bone tumor; nevertheless the diagnosis of malignancy must be done by
