Direct sac puncture approach to expanded popliteal artery aneurysms after exclusion and bypass.

Description
Summary:Expansion of ligated Popliteal Artery Aneurysms (PAAs) is a well-documented entity post bypass surgery. Endoaneurysmorrhaphy and ligation of branches within the sac has been the main treatment for such cases, while there are recent reports of endovascular repair with transfemoral coil embolization. We present two patients with enlarging ligated PAAs treated successfully with direct sac embolization with copolymer embolic agents (Onyx and Squid). The availability of simple endovascular techniques may change the treatment paradigm of ligated PAA expansion in a similar way that type 2 endoleaks post EVAR have been treated in asymptomatic patients with sac growth.
