The case of a 22 year old man with a spontaneous hemothorax and no previous history of trauma, cough, sputum production or fever, is presented. The patient was admitted to the Intensive Care Unit of the Arnaldo Milián Castro University Hospital in Santa Clara city, Villa Clara, Cuba . The diagnosis by pleural biopsy was chronic pleuritis with mesothelial reaction, and the case was considered as an idiopathic spontaneous hemothorax.
