Summary: | Bacteria species of Cedecea, belonging to the Enterobacteriaceae family, is not among common pathogens. An 85 year-old female patient with a history of diabetes and hepatocellular carcinoma due to chronic hepatitis C was admitted to the Medical Oncology Service with complaints including icterus, asthenia, and diarrhea. Owing to the patient’s age and bulk’s place near the portal vein, surgical resection could not be performed. As patient’s total bilirubin raised up to 9.6 mg/dL on the second day of admittance, trans-arterial chemoembolization, and three days later, percutaneous transhepatic cholangiography were applied. Four days after these procedures, her temperature rose to 39°C. There was no growth in the urine and catheter tip cultures of the patient. However, the presence of gram-negative bacterium Cedecea lapagei was documented in both blood cultures by Phoenix (Becton Dickinson, USA) systems. Antimicrobial susceptibility test of the bacteria was studied according to the criteria of European Committee on Antimicrobial Susceptibility Testing (EUCAST). The strain was confirmed as only having moderate sensitivity against tigecycline, and was resistant to all other groups of antibiotics. The patient, receiving tigecycline and meropenem treatment, died on the 25th day of admittance. C. lapagei has been reported as the cause of pneumonia and urinary tract infections until today in our country, and due to the fact that this is the first bacteremia case reported from our country, it is deemed worth presenting. |
