description Introduction The aim of the present study was revise the technical issues of  hip arthroplasty after a failed proximal femur fixation as well as evaluate the results and complication associated with this procedure. Materials and Method We performed 61 hip arthroplasty after a failed intertrochanteric or subrtochanteric fixation. The average age was 76 years (range, 50 to 93 years). Thirty-four (56%) was originally treated with dinamic hip screw (DHS), eight (13%) with dinamic condilar screw (DCS), two (3%) with  Ender nail and seventeen (28%) with  proximal femoral nail.  Fifty-five (90%) patients were treated with total hip arthroplasty and six (10%) with hemiatrhroplasty. In seventeen (28%) patients we used uncemented stem and in forty-four (72%) cemented stem. In 12 patients we used a standar lenght stem and in 49 a long-stem. Results At the time of fisrt year of follow up the HHS impoved from 47 ( range, 32 to 54) to 84 (range, 67 to 93). Seven patients (11.5%) had complications. Three (4.9%) had periprosthetic femoral fracture, two (3.2%) had  dislocation, one (1.6%) developed a wound hematoma and one (1.6%) a deep infection. Conclusion The hip arthroplasty after a failed fixation of a trochanteric or subtrochanteric fractures is  a relaiable method in selected cases. Most of them achived good results in terms of pain relief an functional outcomes. However, is technically challenging and is associated with more complications than a primary hip repleacement.   Key words:
