description Chronic diseases have become a very common health issue among people nowadays. This is caused by their lifestyle, neglecting the common symptoms surfaced from chronic diseases and not having health check-up at least once every two years. This project aim to further highlight the importance health monitoring system and raise the health-conscious of the user. The proposed system is a personal health monitoring application, Personal Health Tracker. The application is simple, interactive and most importantly elderly-friendly. Also, the application allows the user to input their vitals and manage their records. In addition, the location and details about the polyclinic and hospital is available in the application itself, without the need to surf the internet. Last, the application will be developed in Android platform and it will be compatible with all the Android mobile devices. The overall implementation was successful, additional features was added which include user particulars, language preference, pedometer, heart rate monitor and health information.
