This research aims to identify the factors that contribute to the successfull application of fissure sealants at the Dental Health Department Clinic of Yogyakarta Health Polytechnic. Research Methods: This is a case study employing a non-experimental quantitative approach. Subjects were 38 fifth-semester students in Dental Health Departement in the 2011/2012 academic year. The measuring instrument used was a questionnaire survey to measure knowledge, the role of the clinical supervisor, the role of facilities and infrastructure, attitudes, motivation as well as a checklist to measure skills that contribute to the application of fissure sealants at the Dental Health Department Clinics. For purposes data analysis, logistic and multiple regression tests were used. Research Results: Analysis of the degrees of success in the application of fissure sealant showed the following results: 68,4% satisfactory, 31,6% broken. The factors that significantly contributed to the successful application of fissure sealants were knowledge (p=0,001), skills (p=0,001), the role of the clinical supervisor (p=0,020), attitudes (p=0,005), and motivation (p=0,005). The factors that had no significant contribution to the successful application of fissure sealants were the subjects of facilities and infrastructure (p=0,151). Conclusions: 1) The subjects of facilities and infrastructure do not indicate a significant influence on the successful application of fissure sealants at the Dental Health Department Clinics. 2) Increased knowledge and skills, the role of the clinical supervisor, attitudes and motivation enhance the quality of the application of fissure sealants. 3) The skills variable makes the greatest contribution, i.e. 21,131% to the successful application of fissure sealants at the Dental Health Department Clinic of Yogyakarta Health Polytechnic.
