description Thalassemia major is inherited disease, which affect hemoglobin sintesis resulting in chronic anemia. As consequence is lifelong blood transfusion therapy to maintain the adequate hemoglobin level. The regular transfusion leads to iron overload which deposited in many organs. Cardiac morbidity due to iron overload is considered to be primary cause of mortality in thalassemia patient. Iron toxicity causes inhomogenous repolarization due to myocardial or conductive tissue damage. Using ECG recording, this can be seen as variability of QTinterval among ECG-leads. QT-interval dispersion is the form of QT-interval variability analisis. Early detection of cardiac involvement based on QTcdispersion is necessary to be studied. Since advanced equipment such as echocardiography and magnetic resonance imaging have limited availability especially in developing country, electrocardiography (ECG) might be has a role to identify cardiac complication. Method: A diagnostic test of QTc dispersion (QTcD) measured by ECG was performed on children with thalassemia major. Echocardiography was used as standard reference to study the cadiac status, in whicth left ventricle mass index (LVMI) and ejection fraction (EF) were defined as indicators of cardiac morbidity. Receiver operating curve analysis was used to find the cut off value of QTcD in relation to abnormality of LVMI or EF. Results: The Study involved 47 thalassemic participants aged 3 � 17 years. When QTcD was analyzed on the present of low EF, the area under curve (AUC) was 0,618 (CI 95%= 0,456 � 0,780
