Summary:Traumatic brain injury is the most cause of mortality in young people. Hypoalbuminemia was noted in 88%, 52%, and 33% of patients with severe, moderate, and mild head injury precisely. Patients with albumin level < 3,4 g/dL were 2.5 times more likely to die compared with patient with normal albumin levels. The study of hypoalbuminemia as a predictor of mortality in patients with severe head injury in Yogyakarta has not been done The objective of this research is to assess hypoalbuminemia as a predictor of mortality in patients with severe head injury. . This research used retrospective cohort study. Data were collected from medical records of patients hospitalized in DR. Sardjito Hospital, Yogyakarta from January 2008 to December 2009. Code of diagnosis are SO6.4, SO6.5, SO6.6, SO6.7, SO6.8. Subjects were divided into 2 groups: 50% subjects in the hypoalbuminemia and 50% into normoalbuminemia group. We used Chi-quare test for univariate analysis and Logistic regression for multivariate analysis. The Kaplan Meier was used to assess survyval analysis. Total number of target population is 1778, with inclusion in to SO6.4, SO6.5, S06.6, S06.7, dan S06.8 diagnosis is 277 subjects. Number of subjects included was 120 consisted male: female ratio was 2,6:1. Mortality rate is 33,3% and 66,7% alive. Total patients with mild hypoalbuminemia was 35%, moderate 12,5% and severe hypoalbuminemia was 2,5%. Hypoalbuminemia had a significant predictor of mortality in patients with severe head injury 2,958 times compared with normoalbuminemia (95% CI 1,33-6,566
