Summary:Background: In the Kulon Progo Regency, Social Askes Participants can choose Primary Health Care or family physicians as first level provider (PPK I). Participants decision is influenced by the providers characteristics. Objective: This study aims to investigate the characteristics of services that influence Social Askes Participants to choose PPK I. Methods: This study is a qualitative research, case study approach. Location of the study is Puskesmas Galur I in Kulon Progo Regency. The respondents were 7 primary health care patients, 7 family physicians patients, 2 family physicians, team of Non Communicable Diseases Control (PTM) Puskesmas Galur I and Head of Health Development, Health Department of Kulon Progo Regency. Respondents were selected by inclusion criteria. The research instrument was indepth interviews with an interview guide and Focus Group Discussion (FGD). Triangulation method is used to test the validity of the data. Data analysis using transcripts, coding of data is done manually, presentation of data, analysis and conclusions. Results: Patients choose primary health care for services integration and owned clinic team approach in terms of out of hours services, nutrition consultation and services laboratorium, whereas patients choosing a family doctor because of the ability as a case manager, are short waiting times, physician contact person available, access financial, information access, effective patient doctor communication, pharmacy services, referral rates are low, the club hypertension and diabetes as a form of peer group. Conclusion: Integrated primary health care and family physicians supported by PT Askes and Health Department policies, can meet better case managers.
