| Summary: | Background : Provision of medical action information is done by doctor to the patient
which is used to explain the medical action information related about medical action
information which written in the medical action information form. The action which is given
by doctor to the patient is not only delivered verbally but also delivered by text in the
information form. Explanation about information delivery related with action which is must
be given so the patient understand completely about what the doctor explain is. The
patient also has right to choose their self which is formulated as medical action agreement
that is patient right to refuse or accept the action after get enough information from the
doctor. The reality Is, there are still many forms provision medical information which is
incomplete data filling if compared with medical action agreement.
Purposes of the study : To know the plot implementation of provision of medical action
information related with patient rights, to know percentage of completeness filling of
provision of medical action information form and to know the factors that influence
completeness filling of provision of medical action information form.
Method : This research uses descriptive method with qualitative approach and research
design uses cross sectional. Research subject consist of 1 general surgeon, 2 ward
nurses, 1 policlinic nurse, 1 manager assistant in nurse division, 1 installation medical
record supervisor, and 1 medical record worker assembling division. The object of this
research is 40 provision of medical action information forms using bred sampling
technique.
Result : Plot implementation of provision of medical action information is not optimal yet in
the information delivery. Majority of the patients are only focused in safety aspect than the
explanation of the action that will be done. Actually, every patient has a right to get the
information about the medical action that will be done. There is a comparison in
completeness of medical action agreement form filling 80,50% higher than provision of
medical action information form that is 46,41%. From that percentage data, there are
factors that influence it that is human resource(doctor, nurse, and medical record staff) and
the policy that provide about procedure about socialization that has been doing, also there
is no reward and punishment in text form yet.
