| description | Background: Most of birth mother mortality is occured during post-partum period. The length of short time period ofmother stay in the hospital impacts the health education for mother with post-partum period and most of the mother did not received adequate information. The development of discharge planning program since early time hopefully can improve the mother�s readiness to take home. Objective: To identify the influence of discharge planning program to knowledge and readiness of patient discharge with post-partum period in the RSUP dr. Soeradji Tirtonegoro Klaten Method: Quasi experimental with non-equivalent control group were done. Samples were taken through purposive sampling involving 88 post-partum patients in Melati Ward, RSUP dr. Soeradji Tirtonegoro Klaten, with inclusion criterias. Intervention group got the discharge planning development program and the control group got the routine discharge program from that hospital. Readiness of discharge�s patient was assessed using Readiness for Hospital Discharge Scale (RHDS). Result: There were significant different of knowledge between intervention group in discharge planning program using p=0,000. There are significant increase of p=0,007 in readiness for discharge. Knowledge of control group and intervention group show significant difference of p=0,000. The comparison between intervention and control group show that after given discharge planning program the distance value are still the same of p=0,768. Conclusion: There was an increase score of patients� knowledge and discharge readiness after discharge planning development program implemented. |
