According to fixed procedures (SOPs) DKT Hospital Dr. Soetarto Yogyakarta calm returns inpatient medical records, medical records must be returned to the unit medical record no later than 2x24 hours after the patient's home or after the patient leaves the hospital. It is intended that the officers are ready to provide medical record patient medical records at any time if necessary. The study design was cross-sectional. Data collection techniques in this study is the observation, interviews, and documentation Results: The percentage of delay in DKT Hospital Dr. Soetarto Yogyakarta is still great tardiness figure includes the wards Kartika reached 87.2%, reaching 94.9% Kirana wards, wards Midwifery reached 61.3%, reaching 90.2% Husada wards, and VIP wards reached 100%. Factors causing delay, namely human resources, especially the ward nurses understanding about the payback period is less, lack of socialization procedures remain, leading to incomplete documentation of medical records that must be returned to the unit medical record should not be returned due to complete filling in advance and not the assembling activities that do not No special officer in charge of checking the completeness of patient medical records. The impact of the delay complicates the recovery of medical records so slow service speed mmepengaruhi outpatient medical records document provision.
