description Background Elevated blood pressure was associated with high levels of body fat. Identification of elevated blood pressure through BMI and waist/height ratio can become strategy for early detection for hypertension. Objective To identify BMI cut off and waist/height ratio cut off as predictors of high blood pressure in children. Method This was a cross sectional study of 928 children 11 to 16 years old in Yogyakarta. Body mass index (BMI), waist/height ratio and blood pressure were measured using standard technique described by The Fourth Report on the Diagnosis, Evaluation, and Treatment of High Blood Pressure in Children and Adolescents. Receiver Operating Characteristic (ROC) curves were used to obtain cut off for high blood pressure. Result The best cut-off point of BMI to predict of high blood pressure in children was 0.51 with sensitivity and specificity of 82% (CI95%78-86) and 76% (CI95% 67-78) for systolic blood pressure and 82% (CI95% 78-86) and 72% (CI95% 68-76) for diastolic blood pressure. Cut-off point of waist/height ratio to predict high blood pressure was 0.45 with sensitivity of 76% (CI95% 67-78) and specificity of 74% (CI95%71-79) for systolic blood pressure and sensitivity of 76% (CI95% 72-80) and specificity of 70% (CI95% 61-75) for diastolic blood pressure. Conclusion BMI with Z score of 0,51 and waist/height ratio of 0,45 are the best cut offs for predict high blood pressure in adolescents
