| Summary: | Background: The medical records of inactive files on the storage shelf is not always going to be saved, because the number of medical records at the hospital continues to grow. Destruction of medical record file is one of the efforts made by the hospital with the aim of reducing the accumulation of inactive files medical records at the storage space.Hospital Yogyakarta IVMA Islam has an active medical record storage space and do not have the medical records of inactive storage space. Inactive medical records are stored in the active medical record storage space. To reduce the inactive file medical records hospital culling. Purpose: The purpose of this study is to identify the planned extermination activities in Yogyakarta IVMA Islamic Hospital. Research methodology: This type of research is a descriptive study with a qualitative approach with a cross-sectional design. Data collection techniques in this study using wawancaradan study documentation. Subjects in this study were 2 medical records clerk. Objects in this study is the identification of measures planned extermination activities in Yogyakarta IVMA Islamic Hospital. Head of the medical record as a source of triangulation. Results: This study showed that the preparation procedure starts with identifying the remains extermination extermination plan of action that is by knowing sense of annihilation, extermination procedures, assessment files, the stages of destruction, extermination policies, the unit involved in the culling, the theory of the SOP, with formatting SOP proposals annihilation and extermination plot.
