| Summary: | Background: Budget allocation of lokal health insurance at Cirebon Municipality
is consistently decreasing compared to lokal government budget. Budget
utilization is debt to health service providers is increasing.
Objective: To describe perception of stakeholders to needs for premi based
budget in lokal health insurance program at Cirebon Municipality in the payment
of health service package and evaluate needs for budget and its utilization.
Method: The study used single holistic case study design. Research unit was
Cirebon Municipal Health Office. Subject consisted of 12 respondents from lokal
parliament (budget council), Lokal Planning Council and Health Office selected
through purposive sampling and supported with quantitative and qualitative
information obtained through premi calculation and indepth interview with
stakeholders on needs for premi based budget of lokal health insurance program
at Cirebon Municipality.
Result: Cost of service claims occured in three health facilities. Operational cost
of lokal health insurance at Cirebon Municipality in 2011 was less than 5% of
capitation per member per month (PMPM) with the pattern of tariff INA CBG�s
2.0 obtained IDR 8.031 , tariff INA CBG�s 2.1 IDR 8.340 and tariff INA CBG�s
3.0 IDR 9.185. The pattern of tariff INA CBG�s premi obtained from calculation
was IDR 8.433 of versi 2.0 and IDR 8.757 of versi 2.1 and IDR 9.644 of versi 3.0
whereas based on local regulation the tariff in 2012 was IDR 9.065. Needs for
budget based on the pattern of tariff INA CBG�s versi 3.0 premi was IDR
9.292.081.356 and based on local regulation the tariff was IDR 8.732.936.375.
Perception of stakeholders on needs for budget was that stakeholders would
provide budget without service limitation. Thus they were prepared with the
implementation of sosial insurance program.
Conclusion: Stakeholders considered the existence of lokal health insurance
program at Cirebon Municipality should be maintained and impoved in the future
as indicated from the provision of contribution of the lokal government through
allocation for budget up to 15 billion rupiahs in lokal revenue and expenditure
budget 2013. |
