Summary:Background : A hospital formulary has been put in effect since 1994 and until 2012 has been eight revised. One of the problems faced by the hospital are tends not to optimally both to see the process of doctors prescribing and procurement. The formulary system must be well managed by involving physicians and related parties so that physicians are consistent in using the formulary when prescribing. Monitoring and evaluation has not been conducted optimally. Objective : To evaluate the use and medicine compliance of dr. Soedomo Hospital Formulary, medicine compliance with formulary, To determine the role of therapeutic committee and Hospital Pharmacy Instalation to monitoring and evaluation the utilization of dr.Soedomo Hospital Formulary, the role of physicians to utilization of dr.Soedomo Hospital Formulary. Method: The method use in this research is descriptive method with case study research design. Research instrument in the form of Hospital formularies, drug procurement proposal list and indepth interview guide. Results: The average utilization of formulary in 2012 for inpatients was 82%, while for outpatients was 83%. While the percentage medicine compliance with hospital formulary in 2012 was 47% of total drug items planned. Based on indepth interviews with informants monitoring and evaluation of the use of formularies is not optimally. Doctors have not been fully implementated formulary. Conclusion: Formulary utilization in the hospital of dr.Soedomo not yet reached optimally. Some drugs held outside the formulary. Monitoring and evaluation has not been optimally by PFT/IFRS. Physicians tend not to optimally use the existing hospital formulary when prescribing.
