Summary: | Background. Formulary is list of medicine approved to be prescribed in a health
service unit. Brief observation on prescription in 2012 in Prambanan General
Hospital indicated use of formulary in the hospital is effective although there is
prescription that is not served in hospital pharmacy. It may due to the drug is not
available in the pharmacy, so there is problem what the drug is not available in
pharmacy due to not in formulary.
Research objective. To evaluate implementation of formulary in Prambanan
General Hospital in 2012.
Method. It is a case study research using quantitative and qualitative method.
Data to observe percentage of formulary implementation used sample of inpatient
and outpatient prescription from April to December 2012. Research subject
include hospital director, therapy pharmacy committee chief, pharmacy chief,
pharmacist, and doctors. Sample was prescription in 2012. Sample was taken
referring to WHO of 1993 with modification of amount of prescription per time
point of 100 prescriptions in inpatient and outpatient classified into three period :
period one (April, May, June), period two (July, August, September) and period
three (October, November, December) so total was 9 months. This was done due
to the 2012 formulary was enforced since April 2012 so there were 600
prescription observed.
Result: percentage of prescription according to formulary in Prambanan General
Hospital for outpatient and inpatient was 78.86% and 87.47%. Drug selection was
done before entering drug in formulary. Drug planning is based on previous
usage, with procurement of 100% according to formulary.
Conclusion. Use of formulary in Prambanan General Hospital has not been
optimal. To increase implementation of formulary, pharmacy and therapy
committee did dissemination the formulary and revising it. In order to implement
formulary in Prambanan General Hospital drug management and support from
management play vital role particularly on information system that may aid to
improve formulary implementation
