| description | Background of Problems: in the Medical Record department of RSUP DR. Sardjito Yogyakarta, it is known that there is no conformity process of writing diagnose on the polyclinic sheets with entry of data diagnose to the SIMRS, in patients� insurance firm in poly medicine. This percentage of the nonconformity is more than 50% of the one hundred patients who have the analysis, it is consider as a preliminary study. Process of writing suitability diagnoses between the written diagnose on the polyclinic sheet with the diagnoses on SIMRS is very important. It means that process of writing entry data diagnose on the SIMRS should be consistent with the written diagnose on polyclinic sheet, because diagnose is written on the polyclinic sheet is fact of the condition of patient is completed by the treating physician. If process of writing diagnose is inconsistent between diagnose is written on the polyclinic sheet and SIMRS, then the result of the data become invalid that affecting the quality of reporting such as reporting of morbidity, 10 reports of illnesses and data processing. In addition, the reporting of data processing is used as a decision making, patient care management, drug procurement decision, assessment of new project or the preparation of primary preventive public health programs.
Objectives: to know the factors causing inconsistency process of writing diagnose on the polyclinic sheet with entry of data diagnose to the SIMRS in patients insurance firm in poly medicine in the department of RSUP DR. Sardjito Yogyakarta.
Metode of research: the type of the research used is a qualitative description with cross sectional design. The subjects in this research were administrative officers in the poly medicine and reporting officer. Object in this research are the patients� medical record of poly medicine insurance period November 2012 amount 224 sheets and the print out the data diagnosed in patients insurance firm in poly medicine on SIMRS that is 12 sheets in total. The Kretchi table is used to determine the sampling techniques by simple random sampling. The method of data collecting used is the documentation study, observation, and interviews.
Result: the executive of entry data diagnose on SIMRS in poly medicine departement of RSUP DR. Sardjito Yogyakarta is done by administration officers and their implementation is still inconsistent with the provisions of the right that is performed after completion of the examination. The results of the implementation of entry data diagnose on SIMRS are still inconsistent. From 224 patients in insurance of poly medicine have analysis result are 65,62% inconsistent, 7,14% diagnose on the polyclinic sheets are not written (empty) and 27,23% are consistent. The factors caused inconsistency process of writing diagnose on the polyclinic sheet with entry of data diagnose to the SIMRS in patients insurance firm in poly medicine are the absence of written policies and standard operational procedure, yet the job description, and ineffectiveness rut of administrative officers to carry out this job ranging from the task as an administrator officer to perform entry data diagnose on SIMRS |
