description Background Occult Hepatitis B Infection (OBI) is the presence of HBV DNA in liver or circulation without detectable HBsAg. It makes blood transfusion safety as a big issues, despite routine screening for HBsAg. Mutation in a-determinant regio is one of the molecular mechanism underlying OBI. Objectives To investigate the prevalence of OBI related to mutation of a-determinant regio in healthy blood donors at UPTD RSUP Dr.Sardjito Yogyakarta. Methods Serum samples of 150 blood donors negative for HBsAg negative were tested for confirmatory test and other markers such as anti-HBs and anti-HBc. Samples were extracted and amplified with nested-PCR methods then read using electrophoresis. Samples with positive HBV DNA then were sequenced. The viral load was determined by quantitative Real Time-PCR. Result Out of the 150 samples, 18 (12%) were positive for HBV DNA, consist of 9 (50%) mutation within adeterminant regio. Of the 9 samples were positive for mutation within a-determinant regio, 8 (88.9%) were anti-HBc (+)/ anti-HBs (-). Conclusion Prevalence of OBI in healthy blood donors at RSUP Dr. Sardjito Yogyakarta was 12%, and 50% is due to mutation within a-determinant region.
