although, in fact, vaccination and antiviral therapy have
also been invented. More than 600.000 people died every
year as the consequence of acute and chronic Hepatitis B.
Indonesia, as other Asian countries, is a country with
moderate-high prevalence of HBV infection. However,
studies on characteristic of mutations within HBV genome
and their roles in pathogenesis of liver disease in the
HBV infected patients have not been conducted in
Indonesia.
Objective: To investigate the prevalence of basal core
promoter mutation, specifically A1762T/G1764 mutation,
within patient with chronic hepatitis B and advanced
liver disease.
Methods: Cross sectional study involved patients with HBV
surface antigen (HBsAg) positive. The participants of
this study are 40 patients with advanced liver disease,
including liver cirrhosis and carcinoma hepatocelullar,
and 40 patients with chronic hepatitis B. Cases with
positive HBsAg expression for more than six months but
did not fulfill the diagnostic criteria of liver
cirrhosis or HCC, were diagnosed as chronic HBV
infection. Basal core promoter mutation was analyzed by
Nested PCR and direct sequencing method.
Results: The presence of basal core promoter
A1762T/G1764A mutation was observed in 23 (57,5%)
patients with advanced liver disease and 10 (25%)
patients with chronic hepatitis B.
Conclusion: The presence of basal core promoter mutation
is significantly higher within advanced liver disease
patients, compared with chronic hepatitis B patients.
This mutation can be used as a predictive marker for the
progression of liver diseases in Indonesia. |
