Summary: | The applying of INA-CBGs on hospital financing with fracture require medication planning and cost analysis because fracture is a disease that expensive so hospitals can make cost savings and not getting lost. Especially in 2014 carried out the implementation of the national system INA-CBGs. The purpose of this research is to perceive whether time depth treatment of patient with fracture at hospital influenced by patient factors and surgery factors, to conduct cost analysis on patients with fracture, also perceive whether the total cost expense for fracture medication fitting with rate INA-CBGs.
Research type is observational with research cross sectional layout is according to hospital perspective. Data taken by using retrospective method. The research subjects are all of patient in RSUD Panembahan Senopati Bantul within January-December 2011 periode who have fracture in one fracture location, treated in class III with jamkesmas, ORIF (Open Reduction with Internal Fixation) surgery, and complete data. Research variables are patient factors, surgery factors, expense required and outcome result of therapy which is measured from patients treatment time in hospital (LOS). Data analysis done by using Chi-Square, Spearman correlation, and Mann Whitney.
Research result indicated that patient factors such age and sex had not significancy level but surgery factors such surgery location, length of surgery, and delay to surgery had significancy to length of stay in patient with fracture. The average cost of fracture treatment is Rp 4.567.422.2 ± Rp 1.426.742,5 for mild upper limb procedures (M-1-80-I), Rp 5.956.427,4 ± Rp 2.337.127,2 for knee and lower leg except for mild foot procedures (M-1-70-I), and Rp 8.181.788,4 ± 1.271.180,5 for thigh and hip joint except mild mayor joint procedures (M-1-20-I). The average total cost is higher than the rate INA-CBGs and the difference was significant.
