Summary: | The amount of defects in weaving department that ranged between 14 % -16 % of the total
production at PT.PRIMISSIMA result in swelling production costs leading to the reduction in
corporate profits. Semiautomatic manufacturing process make human skill and competence
as central role in proces and product quality. The goal of this research is calculating human
error probability and strategic recommendation to maximize human reliability.
Cognitive Reliability and Error Analysis Method (CREAM) gived classification which used to
analized and predicted human performance based Contextual Control Modes (COCOM).
CREAM Methodology has been selected among the most known and used methods for Human
Reliability Analysis and Common Performance Condition(CPC) approach is considered
appropriate factors that influence the probability of human reliability. The level of eight
CPC assigned from 21 Hani and 64 Air Jet Loom (AJL) operator�s quisionnaire. Fuzzy
method is using to classifying system for human reliability in order to minimize operator
subjectivity. Strategic recommendation based on CPC�s level and number of CPC should
improved which convert in COCOM, HAZOP analysis and operator cognitive function.
CPC that should be improved in Hani are organization, work environment, time constraints,
objectives and organization, procedures, time constraints and objectives in AJL. Both of
them are in opportunistic condition based on COCOM Method with human error probability
Hani is 0,2878 and 0,3508 for AJL. Components which categorized as extreme risk in Hani
are generators, heat pumps, steam pipes, circulating pumps, exhaust pipes starch, starch
exhaust valves, agitator shaft, roller dryers, rollers briber, the adjusting wheel angles, rings,
bolts and clamp beam. AJL components which in the extreme category are feeder drum,
comb, wind regulator, rear axle roller clutch drive. Observed is cognitive function which has
a major role in carrying out the task then followed by the interpretation and execution
