head injury visits to Emergency Room Islamic hospital Ibnusina Pekanbaru and
followed by increase of mortality in the last 4 years. The purpose of this study was
to know the relationship between management of head injuries case in emergency
room with clinical outcome as waiting time of emergency room service, waiting
time of operation, the type of treatment room and length of stay as well as look at
the factors that influence it.
Methods: The study was a prospective observational cohort study design. Data
and information regarding service process were obtained from observation and
document review. Data were collected from October 2012 to December 2012 then
analyzed with SPSS version 21 to get the value of RR with 95% confidence
intervals. Stated effect if p <0.05.
Results: The results obtained 105 head-injured case who visit the ER.
Comparisons between men and women (60%: 40%), aged <18 years (21%) and
age> 18 years (79%). Nearly 70% due to a motorcycle accident. mild HI (60%),
moderate HI (24,8%) and severe HI (15.2%). The proportion of respon time in the
ER < 5 min (56.2%), more than half of the study subjects (51.4%) the clinical
diagnosis time more than 15 minutes, the CT scan waiting time < 60 minutes
(71,2%). There were relationship between comorbidities with ER waiting time (p
= 0.02), a waiting time CT scans with ER waiting time (p = 0.01). The
relationship between time on scene to hospital with waiting time of operation (p =
0.02 and RR: 4.66). The compare between ER respon time < 5 minute vs > 5
minute with waiting time of operation (46.7% vs 50%) with p=1.07. Time on
scene to hospital with type of treatment room (p=0,01 dan RR: 3.38). The
relationship between compliance to SOPs with type of treatment room (p=0,01
dan RR: 3,09) and the relationship between severity of injury with LOS (p=0.01).
There were relationship between compliance to SOPs with LOS (p=0.01 and RR:
2.72).
Conclusions: There is a relationship between comorbidities and waiting time of
CT Scan with duration time in emergency room. Mode of transportation, severity
of head injury and compliance to SOPs is a relationship with LOS and type of
treatment room. There is relationship between time on scene to hospital with
waiting time of operation.
To improve the quality of care in Emergency Unit Islamic Hospital
Ibnusina Pekanbaru it is necessary to develop clinical guidelines and clinical
pathway in head injury case and to do training for emergency room staff. That
requires the active participation of the Medical Committee in control and do
periodic audits at the Emergency Unit.
