description Background: Completeness of the filling of consent is very important because it is one of the aspects that are assessed in standardized accreditation KARS 2012 and as a tool to ensure compliance the hospitals provide the recording back to back. A filled full consent reflects that the right of patient or family has been fulfilled which means they have received the information needed for an act of medicine to be done. Based on preliminary study that the review covered medical related consent ever done before by RSUP Dr. Sardjito has not reached the standard expected for the average completeness is 65.80% while the standard that must be achieved is 80%. Objective: This research aims to know the percentage of completeness consent filling based on closed medical record review hospital accreditation standards 2012 and knowing the factors that cause incomplete filling and the efforts made to overcome the incompleteness filling consent. Methods: This type of research is a descriptive study with qualitative approach and case study research design. Subjects in this study were 3 assembling officers, 2 officers inpatient admission, 2 nurses room and 2 doctors. Objects in this study were 100 files medical records of inpatient patients. Sampling object in this research using purposive sampling. For collecting data techniques using the technique of study documentation, interview and observation. For the validation of data using triangulation sources and triangulation techniques. This research was conducted in the Medical Notes Installation and Inpatient Instalation 1 in RSUP Dr. Sardjito. Results: Based on the closed medical record review of 100 files inpatient medical record there which form consent obtained the results percentage of completeness of the filling standard for consent HPK 6.3 standards related to general approval by 80%, HPK 6.4 standards related approvals and actions invasive surgery by 92% , HPK 6.4 standard blood transfusion-related approvals and blood products by 89%, PAB 7.1 standards associated risks, benefits, complications and alternative operations by 96%. Factors that cause incomplete filling is a flurry of people who filled out the sheet because the amount of jobs that lead to inaccuracy and have not been socialized standard operating procedure (SOP) as a whole. Efforts have been made to overcome the incompleteness of filling is to establish communication and socialization.
