description Background: According to the Regulation of the Minister of Internal Affairs and the Minister of Health No. 15 of 2010 and No. 162/MENKES/PB/I/2010 that any deaths that occur outside of health care facilities should be traced for the cause of death which is conducted using Verbal Autopsy (VA). Based on interviews with SIK staffs Health Surveillance Agency of Yogyakarta, it is observed that VA report has some obstacles ---being late in reporting and incomplete in coding the cause of death. Jetis Primary Care (Puskesmas) Yogyakarta is the one with the highest VA in Yogyakarta, and based on the observation,the implementation of VA is frequently made during clinic services and therefore it disturbs the service. Objective: To evaluate the implementation of VA ranging from interviews to the family of the deceased and then it is sent to the health department if it is in accordance with the available handbook. Methods: Descriptive using a qualitative approach. The design of the study is a cross-sectional design. Data collection techniques in this study are the observation, interview and documentation. The subjects are doctors and nurses who conduct AV. The object is the implementation of VA and Form of Death Cause Description (FKPK). Results: The results of evaluation on the implementation of VA in Jetis Primary Care Yogyakarta show that some of the existing implementationsare in accordance with the handbook but some are not.The proper implementationsare held by VA implementers Jetis Yogyakarta as they havereceived trainings, the determination of death causes have been in accordance with the Ministry of Health, and the results reported by VA implementersin Jetis Primary Care Yogyakarta have been performed as required by the City Health Office Yogyakarta. While the implementation of VA that is not in accordance with the handbook is as follows--- no Work Procedures and Work Instructions (IK / PK) on the implementation of verbal autopsy, no FKPK Form Filling, and no service for the death certificate and what is available is a draft of mechanism on death certificate that has not yet to be authorized, there is no clear standard, there has been different opinion on the provision of respite for families mourning the deceased before the implementation of the VA interview, VA interviewees are not always family of the deceased who knows the history of the disease clearly, the percentage occupancy of encoding mortality causes in Jetis Primary Care Yogyakarta is 37.21%, and the difference in understanding between the health department and health center on the date of report delivery to the health department.
