Implementation of safety climate in healthcare has been done in other countries using a questionnaire. The number of questionnaires were generated from the study have a number of different dimensions according to the countries where the research is done. Cultural differences on the assessment of safety climate in other countries provide different dimensions that form the conditions in Indonesia. Therefore, this study aims to develop a measurement instrument of safety climate / culture, especially in hospital intensive care units as perceived by the medical team and emergency unit also know the general picture of safety climate conditions contained in the hospital. This research was conducted in the emergency room with 3 general hospitals (RSU) in Yogyakarta with 84 respondents consisting of nurses, doctors and midwives who work on the unit. The survey was conducted using a crosssectional survey. Analysts factors using Ekploratori Factor Analysis (EFA). Questionnaires were given to the respondents already show Hosfstede culture. For validation questionnaire was conducted by questionnaire to the different public hospitals with the same research unit The results obtained from this study resulted in a valid and reliable questionnaire for each dimension of the validity of> 0.3 and reliable above 0.6, and also the validity of the questionnaire results against other hospitals produce consistency of psychometric questionnaires. In this study also showed an index level of each dimension to the hospital. General description of each of the hospital safety climate can be described by the level obtained that the RSU and the RSU II I leadership, communication, and teamwork also already have a good level, while the RSU III 3 dimensions at the level of 1 for the dimensions of leadership , personal and communication. Personal dimension of the three hospitals are at level 1 which explains that the personal dimension is still worse in the three hospitals.
