Blood is routinely screened for transfusion transmissible illnesses but not malaria. Screening for transfusion transmitted malaria to exclude blood donations at risk of transmitting infection from donors to recipients is a critical part of the process of ensuring that transfusion is as safe as possible. Direct detection of parasite by microscopic examination is still a gold standard. However, the technique is time-consuming, highly operator-dependent and prone to error. Consequently there is a risk that this approach will not detect lower levels of parasitaemia where transmission may still occur. Malaria rapid diagnostic tests (RDT), in simple kit form, can provide results based on fingerprick or venous blood within minutes and may be better able to identify parasitaemic donations, including those with much lower levels of parasites than are reliably detectable by blood smear. The validity of RDT is various according to antigen target and the trade mark. Objectives: To evaluate the sensitivity of the malaria rapid diagnostic test in comparison with microscopic blood smear for malaria blood donors screening, especially in an endemic area. Methods: A diagnostic test is carried out in the study, an alternative test is compared by a microscopic conventional method as a gold standard. Research is done in Maumere, Sikka District, East Nusa Tenggara. Subject involved in the study are potential blood donors in the Blood Transfusion Unit, TC Hillers Maumere. Venous blood of donors was screened for malaria parasites using rapid diagnostic test and Giemsa-stained and the films examined under the microscope for malaria parasite. Parasite densities were estimated for the malaria positive slides. The result analyzed by descriptive statistic test and 2x2 table comparison test method. Results: Diagnostic test data analysis showed that the rapid diagnostic test revealed 97,4% sensitivity, 100% spesificity, 100% positive predictive value, and 99,8% negative predictive value. Conclusions: It can be concluded that rapid diagnostic test is reliable to be used as a malaria screening for blood donors in endemic area.
